ANALISIS CACAT PADA PROSES PERCETAKAN NOTEPAD DENGAN MENGGUNAKAN METODE FAULT TREE ANALYSIS (FTA) DAN FAILURE MODE AND EFFECT ANALYSIS (FMEA) DI CV. HAREN
Abstract
CV. Haren is one of growing printing company at Padang City. Company produces some printing product such as book, invitation, notepad etc. This research was focused on notepad where defectsoften occurred in the production process. That defect cannot be repair, so that become a reject product. The objectives of this research were identified root cause of defect product by used Fault Tree Analysis (FTA), analyzed potential cause of failure with Failure Mode and Effect Analysis (FMEA) method and proposed of improvement to minimize potential failure used 5W+1H. Result of FTA pointed out 4 failure modes and 11 potential caused of failure. Result of FMEA shows 6 potential failures with highest Risk Priority Number (RPN) for every failure mode. Potential failures of sloppy cuts (RPN 576) were workplace not ergonomic and lack of lighting. Potential failure of skewed print out with RPN 512 was paper clip do not work optimally. Potential failures of scattered ink (RPN 512) were operator does not pay attention of water drains to rollers and machine was old. Potential failure of torn paper (RPN 441) was operator has too jobs to handle. Final results were work instruction including for water drain and paper clips inspection, improvement of cutting table dimension form heights 63 cm to 100 cm based on anthropometric measurement and improvement of lightning from 200 lux to 300 lux based on Ministry of Health Regulation.
