ANALISA MEMINIMALISIR RISIKO DEFECT PADA PEMBUATAN SEPATU MENGGUNAKAN METODE FAULT TREE ANALYSIS (FTA) DAN FAILURE MODE AND EFFECT ANALYSIS (FMEA) (Studi Kasus di Honesti By Yoesani Shoes)

Abstract

Honesty By Yoesani Shoes is a company engaged in the leather shoe manufacturing industry. The types of shoes produced include loafers, casual, boot, special committee and sandals with the main raw material of cowhide and using goat skin if there is a demand for orders from consumers. There are so many risks that may occur during production. One form of risk that is encountered is the risk of defects in shoe manufacturing, types of defects that are often encountered including the use of inappropriate glue so that the glue is visible on the shoe soles, untidy seams and wrinkles on the leather of the shoes. The research was conducted in the last six months in June-November 2020, the company was able to produce 1,592 pairs of shoes and 110 pairs of shoes were found with defects with a percentage of 6.90%. The highest RPN 512 occurred for untidy stitches, RPN 392 occurred in The lowest wrinkles on the skin and the glue were seen on the soles of shoes with RPN 280. Proposed improvements to minimize risk were carried out using the PDCA (Plan, Do, Check and Action) approach.
