This company produces petro mini fuel station called “Pertashop” which consists of main components as modular, canopy and totem. One of the of the most critical components in production failure is the Casing of Pertashop’s Modular. This production failure occurs due to the causal factors that arise during the production process and this research was carried out to find out the cause of production failure. This research was conducted by identifying and analyzing the causes of the production failure of Pertashop Casing components using Failure Mode and Effect Analysis. Based in this method, it’s miles feasible to understand the severity, feasible reasons, and motive detection values for each production failure. Then, the highest potential causes of failure are ranked. Like UKM in general, the physical building of these UKM is integrated or share with the main house building. The current layout of UKM Bintang Purnama is currently become a problem. As the packaging station is separated by the kitchen of the house from the production area, because of this, it’s become a problem for the workers due to the distance apart. So for this reason, research is carried out to improve and design the proposed layout for UKM Bintang Purnama. This research was conducted using the Systematic Layout Planning (SLP) method and the Graph method and then compared the two alternative layouts from both methode. Based on the results of the research, the proposed layout for UKM Bintang Purnama is the proposed layout with the Systematic Layout Planning (SLP) method, because it has the smallest displacement distance, which is 31.7 smaller than the proposed layout with the Graphics method which is 43.5 and for the area of production needs of 142.6 m2. At present there is often a problem of lack of availability of gypsum raw materials in the storage area of the Indarung V factory so that to avoid delays in the production process the factory must receive raw materials from other factories. The problem of inaccurate raw material planning can also be caused by weather factors and transportation factors, so that if it not resolved it will disrupt the smooth production. This research will determine the gypsum raw material inventory planning for a 1-year production period, determine the reorder point and how much capacity the order will be made. Gypsum raw material inventory planning method using the min max method. Based on the result of the study, it was found that the minimum and maximum values of the storage were 3,810 tons and 6,076 tons with an order quantity of 2,266 tons each time. With 8 work accident incidents, August 2020 saw the highest number of work accident cases. The stamping department needs to conduct a possible hazard analysis, risk assessment, and risk control using the Hazard Identification Risk Assessment and Risk Control (HIRARC) technique due to the presence of potential dangers and the high frequency of work accidents. The tooling set up process and the tool down process, both of which have a consequence value of 4 and a likelihood of 4, are the two procedures with Extreme risk levels identified by the analysis at the seven stages of the production process. The controls are modifying the machine, using Personal Protective Equipment (PPE) according to SOP, rearranging the layout machines as well as perform preventive maintenance to ensure the machines and equipment used during tooling set up and tool down are in usable condition. Hence, it is necessary to apply a simulation. Today, research related to transportation simulation is less popular. Meanwhile, several transportation simulation studies do not explain in detail how the model is built into the software used. Therefore, this study explains how to use the Arena software for transportation problems using the Transporter. The designed model is a transportation system that uses a transporter which is useful for transporting goods from location 1 to location 2, then the transporter returns to location 1. X is a company that engaged in the manufacturing and trading industry. This company is make to stock and some are moke to order. Based on work accident data, the statistical value of work accidents is 4.49, which means > 2.00. This is indicating that performance or OSH performance needs to be evaluated on K3 policies and implementation. Occupational accidents can be detrimental to workers such as physical disabilities experienced by workers, as well as losses to PT. X, from lost workdays. This research is a qualitative descriptive study. The data collection technique uses structured interviews with the person in charge of OSH implementation as primary data and document evaluation as secondary data. From the data processing it was concluded that work accidents often occur. Currently the company is carrying out the production of new products in large quantities and in a relatively short time, so it is necessary to prepare JSA for these new products. To ensure that the project can be carried out in accordance with the provisions of the maximum according to the provisions of the time and in a managerial context it can be carried out more effectively and efficiently, the first step by the project implementer is to determine and ensure when the project starts and ends its implementation. One of the methods used to ensure this is the Critical Path Method (CPM). This article attempts to apply this method to ensure that the 35,650 meter fiber optic network installation project can be implemented successfully. The results show that the project implementation can be completed 14 days faster than the predetermined time with an efficiency of 38.67%. Eva Suryani M.
